Discussion

  • Rural Health Transformation Program (RHTP)
    • Wyoming was awarded $205million for first round of RHTP funds
      • Wyofile article here (Published 12/31/2025)
    • Recently, Governor Gordon signed a bill enacting the RHTP into law.
      • View the bill here
      • View a WPM article about the bill here
    • Currently, Wyoming is waiting to hear for final approval from CMS, which will trigger the release of Requests for Applications and Proposals (Anticipated in April).
      • Given the scope of the projects listed in the bill, and the speed of application turnaround, we feel this is a great opportunity for organizational alignment to reduce redundant applications/proposals.
    • Several projects align with WyTN’s goals of improving access to care
      • As you brainstorm/prepare applications, please consider WyTN for the telehealth perspective and partnership opportunities.
      • Reach out to other organizations you feel should be involved in the RHTP/Community Health conversation and invite them to this SIG.
  • EMS/Telehealth Practices in Wyoming?
    • There is Community Health Integration/ Chronic Care Management program in Sublette hosted through their EMS/paramedic program
      • Conduct home visits/medicine management etc
      • Not sure if they’re utilizing TH for consultations or general appointments.
    • Campbell County is looking into a similar program to Sublette.
    • Sweetwater has considered it but has questions surrounding billing for CCM vs Home Care.
      • Additionally, EMS organizations that are not owned/operated by the hospital may have different considerations.
