The Community Health and Chronic Disease Prevention (CHCDP) Special Interest Group seeks to improve access to telehealth in communities across the state by identifying and filling gaps in care services, and exploring ways to reduce emergency medical care through preventative measures.

– This Project ECHO network aims to build skills, share expertise, and strengthen collaboration among delivering providers in rural or resource-limited settings.
– Who should attend: Family Medicine Providers, OB/GYNs, ER Physicians, CNMs, PAs, NPs, nurses, and any clinician caring for pregnant patients

– WDH is currently working with contractors such as Mountain Pacific to develop a Steering Committee. - CCHP + WITH Foundation
– The Center for Connected Health Policy partnered with the WITH Foundation to develop TH resources for serving patients with IDD. The resources were created for patients and providers specifically, however, implementing accessible TH Practice helps everyone!

– Group is watching/waiting for updates. RHTF notice of award will be delivered on Dec 31st, and funds are expected to roll out quickly thereafter.
