• Wyoming 211/ CommuniCare
  Ann Clement will be delivering a presentation about CommuniCare at the next CHCDP SIG meeting.
  We are planning for time at the end for Q/A, however, if you wish to email questions to Kristen (kglennie@wyo.edu) questions before hand -please do so! I will share those I receive with Ann so she is able to prepare in advance.

• UW ECHO in Rural Obstetrical Care
  – This Project ECHO network aims to build skills, share expertise, and strengthen collaboration among delivering providers in rural or resource-limited settings.  
  – Who should attend: Family Medicine Providers, OB/GYNs, ER Physicians, CNMs, PAs, NPs, nurses, and any clinician caring for pregnant patients

• The Wyoming Diabetes Summit – Save the Date!

• Preparation for the Social Determinants of Health (SDOH) Conference are under way
  – WDH is currently working with contractors such as Mountain Pacific to develop a Steering Committee. 
• CCHP + WITH Foundation
  – The Center for Connected Health Policy partnered with the WITH Foundation to develop TH resources for serving patients with IDD. The resources were created for patients and providers specifically, however, implementing accessible TH Practice helps everyone!

• Legislative session and Rural health Transformation Fund
  – Group is watching/waiting for updates. RHTF notice of award will be delivered on Dec 31st, and funds are expected to roll out quickly thereafter. 
  – Linked below is an article describing a ‘blueprint’ for telehealth implementation with the RHTF.
  – Please reach out to WyTN for partnership/collaborative opportunities related to the RHTF projects! 

• Telehealth Impact Stories 
  – Collecting patient stories to mail to legislators. Our goal is to create a graphic before the legislative session starting in February.
  – Stories will be de-identified and may be used thematically.
